Good Faith Estimate
Effective date: August 11, 2026. Ignite HRT does not bill insurance, so federal law gives every patient the right to a written estimate of expected charges before care. This notice explains that right and how to use it.
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.

How this applies at Ignite HRT
Matthew Brown ND, LLC, doing business as Ignite HRT, does not bill insurance. Every patient we see is treated as a self-pay individual under the federal rule, so the right described above applies to you whether or not you carry health coverage. Our published prices are the starting point for your estimate: the new-patient and consultation fees listed on our scheduling page, and the monthly membership pricing for ongoing care.
Your written estimate reflects the visit or program you are scheduling and the items and services we reasonably expect to provide with it. Baseline laboratory work included with a new-patient HRT or TRT visit is part of that visit’s published fee. Medication, additional or specialty testing, supplies, and services from outside providers are priced separately where our published pricing says so, and they are discussed with you before they are ordered. HSA and FSA cards are accepted for eligible expenses.
How to ask for your estimate
You can ask for a Good Faith Estimate before you schedule anything, and you can ask for one at any time afterward. Call or text 503-925-6605, email info@ignitehrt.com, or ask our team when you book. Under the federal rules we provide it in writing, on paper or electronically, whichever you prefer:
- Within three business days when you request one without scheduling.
- Within one business day when you schedule a visit at least three business days in advance.
- Within three business days when you schedule a visit at least ten business days in advance.
What the estimate does and does not cover
- It covers the non-emergency items and services we reasonably expect to provide for the visit or program you asked about.
- It is an estimate, not a bill and not a contract. It does not obligate you to receive care, and it does not by itself establish a clinician-patient relationship.
- It does not cover items or services that could not reasonably be expected at the time it was written, such as testing added after an evaluation or care you choose to receive from another provider.
- It does not apply to emergency care. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
If your bill is higher than your estimate
Tell us first. Call 503-925-6605 or email info@ignitehrt.com and we will review the charges with you. You can ask us to update the bill to match the estimate, ask to discuss the bill, or ask whether any assistance is available.
You may also start a patient-provider dispute resolution process with the U.S. Department of Health and Human Services when your bill is at least $400 more than your Good Faith Estimate. You must start that process within 120 calendar days of the date on your bill, and an administrative fee applies; the current amount and the forms are published at cms.gov/nosurprises. Disputing a bill will not affect the quality of the care you receive from us.
