Health Information Privacy

Effective date: August 11, 2026. This page explains how Ignite HRT keeps patient health information confidential — where your records are kept, the laws that protect them, and the choices you have.

Our commitment

Hormone care is personal. The symptoms you describe, the labs you run, and the treatment you choose are between you and your care team, and keeping them private is part of the care itself. Matthew Brown ND, LLC, doing business as Ignite HRT, treats the health information created at our Gresham and Beaverton clinics as confidential, and this page describes what that means in practice.

Where your records are kept

Your patient records live in CharmHealth, the secure electronic health record and patient-portal system our clinics use. Access is limited to Dr. Brown and the staff who need the information to provide your care, manage scheduling and records, or handle payment. The patient portal gives you your own window into your chart, protected by your login.

The laws that protect your information

Oregon law protects the confidentiality of health information held by Oregon health care providers, and we follow the confidentiality standards of ORS 192.553 to 192.581 in how we handle yours. Under those standards, we use and disclose your health information to provide and coordinate your care — for example, with a laboratory or pharmacy involved in your treatment — to obtain payment, and to run the practice, along with other uses and disclosures that Oregon law permits or another law requires, such as public-health reporting or responding to a valid court order. Outside those situations, our policy is to require your signed authorization before disclosing your health information.

Oregon's breach-notification law, the Oregon Consumer Information Protection Act, also applies to us. If a security breach ever compromised your personal or health information, we would notify you promptly, as that law requires.

If another law gives particular health information greater protection — some mental-health, substance-use, HIV- or STI-related, genetic, and reproductive-health information receives extra safeguards — we follow the more protective rule.

What we do not do

  • We do not sell patient health information.
  • We do not give your health information to third parties for their advertising or marketing.
  • This website carries no advertising pixels and no third-party marketing analytics scripts — including on the scheduling page — and we intend to keep it that way.

Your access and choices

  • See your records. The patient portal gives you direct access to much of your chart, and you can request copies of your records from us. Oregon law limits the fees a provider may charge for copies.
  • Ask for a correction. If something in your record looks incorrect or incomplete, tell us and we will review it with you.
  • Choose how we contact you. You can ask us to reach you at a particular phone number or address, and we will accommodate reasonable requests.
  • Decide who else is involved. Tell us whether we may discuss your care with a family member or anyone else involved in it, and we will follow your preference whenever you are able to state it.

A note about email and text

Ordinary email and SMS may not be encrypted. Please keep diagnoses, medication details, and other sensitive health information out of everyday email and text — the patient portal is the right channel for those conversations, or call us and we will find a secure way to help. Do not use this website, email, text, or portal messaging for emergencies; call 911.
